How to Complete the Application for Licensing of Motor Vehicle Form at NaTIS

Section 1: Particulars of Owner

• Type of Identification: Mark with “X” next to the appropriate type of identification, such as National ID, foreign ID, or business registration number.
• Identification Number: Enter the identification number associated with the type of identification selected.
• Country of Issue (if foreign ID): If a foreign ID is selected, enter the country where it was issued.
• Surname/Name of Organization: Enter the surname or name of the organization that owns the motor vehicle.
• Initials and First Names: Enter the initials and first names of the owner, not exceeding three (3).
• E-mail Address: Provide a valid e-mail address for communication purposes.
• Telephone Number at Home: Enter the telephone number where the owner can be reached at home, including the area code.
• Contact Telephone Number during the Day: Enter a contact telephone number where the owner can be reached during the day, including the area code.
• Facsimile Number: If applicable, provide a facsimile number for communication purposes.
• Cellphone Number: Provide a cellphone number for communication purposes.
• Postal Address: Enter the postal address where correspondence can be sent.
• Suburb: Enter the suburb where the owner resides.
• City/Town: Enter the city or town where the owner resides, including the postal code.
• Street Address: If different from the postal address, provide a street address where the owner resides.
• Address where Notices must be Served: Mark with “X” next to either postal address or street address as the preferred address for serving notices.

Section 2: Organization’s Proxy

• Type of Identification: Mark with “X” next to the appropriate type of identification for the organization’s proxy.
• Identification Number: Enter the identification number associated with the type of identification selected.
• Country of Issue (if foreign ID): If a foreign ID is selected, enter the country where it was issued.
• Surname and Initials: Enter the surname and initials of the organization’s proxy.

Section 3: Organization’s Representative

• Type of Identification: Mark with “X” next to the appropriate type of identification for the organization’s representative.
• Identification Number: Enter the identification number associated with the type of identification selected.
• Country of Issue (if foreign ID): If a foreign ID is selected, enter the country where it was issued.
• Surname and Initials: Enter the surname and initials of the organization’s representative.

Section 4: Identification of Motor Vehicle

• Licence Number: Enter the current licence number of the motor vehicle.
• Vehicle Register Number (if available): If available, enter the vehicle register number.
• Chassis Number/VIN: Enter the chassis number or VIN (Vehicle Identification Number) of the motor vehicle.
• Make: Enter the make or manufacturer of the motor vehicle.
• Series Name (describe in full): Provide a detailed description of the series name of the motor vehicle.
• Odometer Reading (if available): If available, enter the current odometer reading of the motor vehicle in kilometers or hours, or indicate if there is no odometer.
• Position of Steering Wheel: Mark with “X” next to the appropriate position of the steering wheel, whether it is drawn, on the left, in the center, or on the right.

Section 5: Declaration

• Read and understand the declaration carefully.
• If you are the owner, organization’s proxy, or organization’s representative, sign in the appropriate space.
• Provide the place and date of signing in the designated spaces.

• This section is for office use only and should not be filled out by the applicant.
