Government Application for Employment Requirements

Before proceeding with the application, it is important to note the following requirements:

  1. The form must be completed by the applicant in full, unless it is not applicable.
  2. A Curriculum Vitae (CV) must be attached by all applicants.
  3. Certified copies of educational certificates and identification documents must be attached.
  4. The Health Questionnaire must be completed in full and attached.
  5. Mark with an “X” where appropriate.

1a. Surname (in Block Letters): Enter your surname in block letters, and if applicable, your maiden name.

1b. First Names (in Block Letters): Enter your first names in block letters.

  1. Namibian Identity Number: Enter your Namibian identity number in the space provided.
  2. Date of Birth: Enter your date of birth in the space provided.
  3. Passport No. and Citizenship (if applicable): If you have a passport, enter the passport number and your citizenship information in the spaces provided.
  4. Work Permit No. (if applicable): If you have a work permit, enter the work permit number in the space provided.
  5. Postal Address: Enter your postal address in the space provided, including the postal code.
  6. Residential Address: Enter your residential address in the space provided, including the postal code.
